In the event of an emergency, I authorize any temple staff, clergy, or event chaperones to obtain emergency medical treatment for my child/children if such persons deem it necessary. I understand that I will be contacted immediately. I also understand that any expense related to emergency medical treatment will be my sole responsibility. In the case a child who requires the use of an Epi-Pen, that will qualify as an emergency and the protocol above will be followed. Please note, Temple Beth El Religious School staff will NOT administer any non-emergency medication to a student under any circumstances.

Parents may ask for a class list (including parent name(s), home address, email address and phone number) in order to contact other parents/classmates. May we share your contact information with other families?

I give my permission for my child/children to be photographed during Religious School programming and any temple activities/services from July 1, 2024 to June 30, 2025. Photographs may be used for promotional material for Temple Beth El programs and for use on the temple website and password protected social media.
